RESUMEN Antecedentes: Aun en estado de cuarentena, las patologías de urgencia de Cirugía General requieren una evaluación y resolución inmediata. Objetivo: Analizar los ingresos, los resultados quirúrgicos y la gravedad de la enfermedad en los ingresos sin COVID-19, durante los primeros meses de la pandemia. Objetivos secundarios, subanálisis de patologías quirúrgicas habituales, como apendicitis, enfermedad de las vías biliares, diverticulitis y tumores complicados. Materiales y métodos: Se realizó un estudio observacional, ambispectivo, sobre una cohorte prospectiva de pacientes que consultaron por patología quirúrgica de guardia y requirieron hospitalización entre el 20 de marzo y el 31 de julio de 2020 correspondiente al Grupo Pandemia (GP), analizados y comparados con una cohorte del mismo período de 2019, Grupo Control (GC). Resultados: En el GP, en comparación con el GC, se registró un aumento en el número de pacientes ingresados (346 versus 305, p = 0,157), un aumento en el porcentaje de casos moderados-graves (58,1% versus 48,8%, p= 0,018), en el tiempo transcurrido desde el inicio de la presentación de los síntomas hasta la consulta (48 versus 24 horas, p< 0,001, en el tiempo desde el diagnóstico hasta la cirugía (23 versus 7 horas, p< 0,001), y en el tiempo operatorio (75 versus 60 minutos, p< 0,001). No hubo diferencias significativas en las complicaciones posoperatorias. Conclusión: Durante la pandemia se observaron cambios desfavorables en las características de la población analizada en relación a la pre pandemia, pero sin impacto en la morbimortalidad posoperatoria, demostrando que fue posible mantener los resultados quirúrgicos a pesar del retraso en la consulta.


ABSTRACT Background: Emergency general surgery conditions require immediate evaluation and timely resolution, even during quarantine. Objective: to analyze variations in admissions, surgical outcomes and severity of the disease in non- COVID-19 emergency admissions during the first months of the pandemic. Secondary objectives, to performed a sub-analysis of common surgical conditions, as appendicitis, biliary tract disease, diverticulitis and complicated tumors. Material and methods: An observational, ambispective study was carried out on a prospective cohort of patients who consulted with on-call surgical pathology and required hospitalization from March 20th, 2020 until July 31, 2020 referred to as Pandemic Group (PG), analyzed and compared with the same period of 2019, Control Group (CG). Results: We experienced an increase in the number of patients admitted at the ED during the pandemic PG vs CG (346 versus 305, p 0.157). Patients in the PG were found to be significantly more ill (58.1% versus 48,8%, p 0.018). A significant delay was found globally in both, time from onset of symptoms presentation to consultation (48 hours versus 24 hours, p < 0.001), and time from diagnosis to surgery (23 hours versus 7 hours, p < 0.001) in the PG, and an increase in the mean operative time (75 versus 60 minutes, p= 0,001). There was no significant difference in postoperative complications. Conclusion: During the pandemic we observed an increase in urgent surgical procedures due to biliary pathology and gastrointestinal tumors. The patients consulted with more advanced stages of disease, but this had no impact on postoperative morbidity or mortality, demonstrating that it was possible to maintain surgical outcomes despite delayed consultation.

INTRODUCTION: Endomedullary nailing using the infrapatellar approach (IP) is considered the gold standard for the treatment of tibial diaphyseal fractures, however, it has been associated with greater difficulty in reduction and complications such as malalignment in procurvatum and anterior knee pain. The suprapatellar approach (SP) arises as an alternative to solve these aspects, also being associated with a shorter intraoperative time and a lower dose and fluoroscopy time. MATERIAL AND METHODS: Retrospective comparative study between a group of 22 fractures treated by SP approach and another of 30 fractures intervened by IP transtendinous approach. Perioperative variables were analyzed, as well as clinical, radiological, and functional aspects in outpatient visits at 3, 6 and 12 months. RESULTS: No differences were found between groups in terms of intraoperative time, anemization, quality of reduction or complications during follow-up, among others. At 12 months, 12 (54.5%) SP cases and 16 (53.3%) IP presented anterior knee pain, without significant differences. In the evaluation scales, significant differences were recorded in the IKDC (International Knee Documentation Committee) in favor of the SP technique 88.45 (76.44-91.1) vs. IP 69 (49.95-80) (p=.006), with no significant differences in other functional scales analyzed. CONCLUSIONS: According to what has been described so far in the literature, the present study supports the tendency toward SP nailing by improving the functional results (IKDC) in the medium term compared to the traditional IP technique, without increasing complications. Likewise, surgeons perceive greater technical ease for reduction and simplicity in obtaining intraoperative radiological images.

Introducción: El enclavado endomedular mediante abordaje infrapatelar (IP) se considera el gold estándar del tratamiento de las fracturas diafisarias de tibia, sin embargo, se ha asociado a mayor dificultad para la reducción y complicaciones como la mala alineación en procurvatum y al dolor anterior de rodilla. El abordaje suprapatelar (SP) surge como alternativa para solventar estos aspectos, asociándose también con un menor tiempo intraoperatorio y menor dosis y tiempo de fluoroscopia. Material y métodos: Estudio retrospectivo comparativo entre: un grupo de 22 fracturas intervenidas mediante abordaje SP y otro de 30 fracturas intervenidas por abordaje IP trastendinoso. Se analizaron variables perioperatorias y aspectos clínicos, radiológicos y funcionales en consulta a los 3, 6 y 12 meses. Resultados: No se encontraron diferencias entre grupos en cuanto a tiempo intraoperatorio, anemización, calidad de la reducción, o complicaciones en el seguimiento, entre otros. A los 12 meses, 12 (54,5%) casos SP y 16 (53,3%) IP presentaban dolor anterior de rodilla, sin diferencias significativas. En las escalas de evaluación, se registraron diferencias significativas en la IKDC (International Knee Documentation Committee) a favor de la técnica SP (88,45 [76,44-91,1] vs. IP 69 [49,95-80]) (p=0,006), sin diferencias significativas en otras escalas funcionales analizadas. Conclusiones: En consonancia con lo hasta ahora descrito en la literatura, el presente estudio apoya la tendencia hacia el enclavado SP por mejorar los resultados funcionales (IKDC) a medio plazo respecto a la técnica tradicional IP, sin aumentar las complicaciones. Asimismo se percibe por los cirujanos una mayor facilidad técnica para la reducción y sencillez en la obtención de imágenes radiológicas intraoperatorias.(AU)

There are few published real-world studies on hepatitis C in Latin America. This paper describes a cohort of Colombian subjects treated with direct-acting antiviral agents. A total of 195 patients from 5 hepatology centers in 4 Colombian cities were retrospectively studied. For each patient, serum biomarkers were obtained, and Child-Pugh, MELD, cirrhosis and fibrosis stage were calculated. Additionally, viral load was quantified at initiation, end of treatment and at 12 weeks of completion. Adverse effects were recorded. Patients with liver transplant were compared with non-transplanted patients in terms of serum biomarkers. The patients had received 9 different regimes. The most prevalent viral genotype was 1b (81.5%). Overall, 186 patients (95.4%) attained sustained virologic response. When comparing transplanted vs. non-transplanted patients, those in the non-transplanted group were more likely to have cirrhosis (52.6% vs. 12.5%, p = 0.0004). Pre-treatment viral load was higher in the transplant group (1 743 575 IQR = 1 038 062-4 252 719 vs. 345 769 IQR = 125 806-842 239; p < 0.0001) as well as ALT and AST levels (82.5 IQR 43.5-115.5 vs. 37.0 IQR = 24.7-73.3; p = 0.0009 and 70 IQR = 41-140 vs. 37 IQR = 24-68; p = 0.004 respectively). Adverse events were reported by 28.7% of the patients; asthenia (5.6%) was the most prevalent. Our results are comparable with those from other countries in terms of therapy and biomarkers. However, our cohort reported less adverse events. Further research is needed in the region.


Existen pocas publicaciones de evidencias del mundo real sobre hepatitis C en América Latina. En este estudio presentamos una cohorte colombiana de pacientes tratados con agentes antivirales de acción directa. Fueron analizados retrospectivamente 195 pacientes seleccionados en 5 centros de hepatología en 4 ciudades de Colombia. Dos tercios fueron mujeres y la mitad tenía ≥ 62 años. De cada uno se cuantificaron biomarcadores séricos, escala de Child-Pugh, MELD y grado de cirrosis y fibrosis. Se cuantificó carga viral al inicio, al final y a las 12 semanas después de completado el tratamiento. Se comparó la frecuencia de efectos adversos de medicamentos en trasplantados vs. no trasplantados. Los pacientes recibieron 9 esquemas de tratamiento diferentes. El genotipo más prevalente fue 1b (81.5%). La respuesta viral sostenida fue alcanzada por 186 pacientes (95.4%). El grupo no trasplantado tenía mayor frecuencia de cirrosis (52.6% vs. 12.5%, p = 0.0004). En los trasplantados, la carga viral pre-tratamiento era mayor (1 743 575 IQR = 1 038 062-4 252 719 vs. 345 769 IQR = 125 806-842 239; p = < 0.0001) igual que la ALT y la AST (82.5 IQR 43.5-115.5 vs. 37.0 IQR = 24.7-73.3; p = 0.0009 and 70 IQR = 41-140 vs. 37 IQR = 24-68; p = 0.004 respectivamente). El 28.7% refirió efectos adversos, siendo el más prevalente la astenia (5.6%). Nuestros resultados fueron comparables a los de estudios publicados en términos de terapia y biomarcadores pero nuestra cohorte presentó menos efectos adversos. Se requiere más investigación en la región.

ANTECEDENTES: La escoliosis idiopática del adolescente se define como una deformidad tridimensional de la columna vertebral que se presenta entre los 10 y los 18 años, y que se manifiesta con una curvatura vertebral en el plano coronal mayor de 10°. Esta deformidad afecta al 2-3% de la población general, pero solo el 10% del total requerirá en algún momento tratamiento quirúrgico. El método de elección para el manejo es el uso de tornillos transpediculares y barras desrotadoras. OBJETIVO: Realizar un análisis descriptivo de los pacientes que recibieron manejo quirúrgico en nuestro instituto con tornillos transpediculares y barras. MÉTODO: Se trata de un estudio observacional, retrospectivo, analítico, abierto, de muestreo no probabilístico, en el que se incluyeron los pacientes tratados con manejo quirúrgico entre 2012 y 2013. Las deformidades se estratificaron de acuerdo con la clasificación de Lenke. El ángulo de corrección de la deformidad, los niveles instrumentados, el sangrado transquirúrgico y la presencia de complicaciones fueron las variables analizadas. RESULTADOS: La mayoría de los pacientes presentaron curvas Lenke IBN, Nash Moe III, Cobb un promedio de 59.4° y cifosis de 47.8. En promedio se siguió a los pacientes por 35.84 meses, detectando un aumento de la curvatura coronal de 2.28° y un aumento de la curvatura sagital de 2.8°. CONCLUSIÓN: Al comparar estos resultados y la literatura mundial se concluyó que el tratamiento de la escoliosis idiopática del adolescente es un método seguro y reproducible que ofrece una mayor ventaja biomecánica y biológica sobre el uso de instrumentación mixta utilizada anteriormente. BACKGROUND: Adolescent's idiopathic scoliosis is defined as a three-dimensional deformity of the spine, which occurs between 10 and 18-year-old, has a spinal curvature >10° in the coronal plane. This deformity affects 2­3% of the general population, however, only 10% of the total will require surgery at some point. The method of choice for management is the use of pedicle screws and rods derotational. OBJECTIVE: To perform a descriptive analysis of patients who received surgical treatment in our institute with pedicle screws and rods. METHODS: This is an observational, retrospective, analytical, open study, non-probability sampling, in which patients requiring surgical treatment at our institute between 2012 and 2013 were included, the deformities were stratified according to the classification of Lenke. The angle of deformity correction, instrumented levels, amount of bleeding, presence of complications were the variables analyzed. RESULTS: Lenke classifying mostly IBN, Moe Nash III, an average of 59.4° Cobb and kyphosis of 47.8. On average it was followed patients for 35.84 months, detecting an increase 2.28° coronal curvature and sagittal curvature increase of 2.8°. CONCLUSION: Comparing these results and world literature concluded that the treatment of adolescent's idiopathic scoliosis is a safe and reproducible method that provides greater biomechanical and biological advantage over the use of mixed instrumentation used previously.

Resumen Introducción El esguince de tobillo es una patología frecuente y uno de los principales motivos de incapacidad laboral, a pesar de ello, no se cuenta con un tratamiento estandarizado y existe controversia con respecto a la inmovilización o una movilización temprana. Objetivo Determinar el mejor tratamiento para los esguinces de tobillo grado II, ofreciendo un resultado satisfactorio con periodos de incapacidad más cortos. Material y métodos Ensayo clínico-aleatorizado, ciego simple de enero a junio de 2016. Dos grupos: inmovilización y vendaje funcional. Se interrogó dolor inicial y al término del tratamiento con escala de EVA, ambos grupos con mismo manejo analgésico, antiinflamatorio y rehabilitación, se evaluó funcionalidad al mes del inicio del padecimiento con escala de Karlsson. Resultados Todos los pacientes con dolor inicial arriba de 70 (de moderado a severo), sin diferencia en media entre grupos (férula 86.3 ± 9.6 contra vendaje de 86.1 ± 8.5; p = 0.95), ambos con disminución del dolor; el grupo con vendaje obtuvo niveles más bajos (férula media 55.2 ± 15.5 contra vendaje 40.3 ± 12.8, p ≤ 0.001). Grupo con férula 24 (88.9%) al final calificaron su dolor como moderado y tres (11.1%) como leve contra el grupo con vendaje, de los cuales, 17 (73.1%) tuvieron dolor moderado y siete (26.9%) leve (p = 0.15). Conclusión El vendaje funcional es una excelente opción de manejo para esguince de tobillo grado II, ofrece buenos resultados con periodos de incapacidad y costos más reducidos.


Abstract Background The grade II ankle sprain it is one of the most frequent pathologies in the musculoskeletal system and one of the principal motives of laboral inability. Despite the fact this pathology is one of the most common reasons for ER visiting there is not a standard treatment for it, and exist controversy about the immobilization vs. an early mobilization. Objective Determine the best treatment for grade II ankle sprains, offering a satisfactory result with a minor time of labor inability. Material and methods Single-blind randomized controlled clinical trial with two groups immobilization versus functional bandage, both groups with VAS evaluation at the beginning and at the end of treatment, same AINES management and rehabilitation, 30 days after the injury we evaluate the functional outcome with Karlsson score. Results At the beginning all patients report pain over 70 (moderate-severe) without difference in the average of both groups (immobilization 86.3 ± 9.6 versus functional bandage treatment of 86.1 ± 8.5; p = 0.95). Both groups present less pain at the end of the treatment, the bandage group present lower values, (Immobilization average 55.2 ± 15.5 versus functional bandage treatment 40.3 ± 12.8, p ≤ 0.001). In the immobilization group 24 (88.9%) at the end of the treatment had moderate pain and 3 (11.1%) light pain against the functional treatment group where 17 (73.1%) had moderate pain and 7 (26.9%) light (p = 0.15). Conclusion Functional bandage is an excellent treatment option for grade II ankle sprain, offering good results with a lower time of inability and cost.

Resumen Introducción: La depresión es la condición de salud que genera mayor carga de enfermedad, donde los adultos mayores manifiestan un gran impacto asociado a esta enfermedad. Este artículo busca describir el impacto que tienen los síntomas depresivos en la funcionalidad de la persona mayor y comprender la relación existente entre la pérdida funcional y la depresión. Metodología: Se realizó una revisión de la literatura de artículos publicados entre los años 1990 y 2016. Se realizó un análisis narrativo de la evidencia encontrada, incluyendo un total de 40 artículos. Resultados: Una de las repercusiones más importantes es la pérdida de la funcionalidad y reducción de la participación en actividades de la vida diaria, asociándose a una dependencia progresiva, mayor riesgo de caídas y temor a caerse, restricción y reducción en la participación en actividades de interés, y peores resultados en procesos de rehabilitación. De esto se desprende la necesidad de entender los síntomas depresivos en conjunto con las repercusiones funcionales en el adulto mayor, comprendiendo que existe una relación recíproca entre la capacidad que tiene la persona mayor para realizar sus actividades diarias y la presencia de síntomas depresivos. Conclusión: Considerar el impacto clínico y psicosocial que tienen la depresión en la funcionalidad de la persona mayor podría contribuir al entendimiento de los síntomas depresivos como un problema de impacto global que requiere de un abordaje integral.

The article describes the impact of depressive symptoms in the functional performance of older adults and seeks to understand the existing relationship between functional impairment and depressive symptoms. Method: a literature review was conducted of articles published between years 1990 to 2016. A narrative analysis was conducted with the finding literature. 40 articles were included. Results: One of the main repercussions is the functional impairment and the reduction in the daily life activities participation, related to a progressive dependency, higher fall risk and fear to falls, restriction and reduction of participation in meaningful activities, and worst rehabilitation and health care outcomes. Considering these elements, it is necessary to understand depressive symptoms in older adults in conjunction with functional impairments, incorporating the reciprocal relationship between older adult's capability to perform daily life activities and depressive symptoms manifestation. Conclusion: to consider the clinical and psychosocial impact of depression on functional performance of older adults could be a contribution in the understanding of depressive symptoms as a global impact problem that requires a multidisciplinary approach.

Knowledge of the best way to evaluate changes in transverse dimension produced by orthodontic treatment enables the estimation of potential space that may be generated and optimizes planning. The aim of this study was to compare two methods for measuring changes in transverse dimension following tooth alignment by passive selfligating system: Method 1 (M1) considering palatal landmarks and Method 2 (M2) considering dental landmarks. Subjects were 12 patients of both sexes, aged 15 to 24 years old, with moderate crowding (Nance discrepancy 4 to 6 mm), who were treated with the Damon System at the Department of Orthodontics of the School of Dentistry, Buenos Aires University. Their preand posttreatment maxillary casts were analyzed (24 casts altogether). Five measurements were taken on each cast: intercanine width, first and second interpremolar width and first and second intermolar width, using the two measurement methods (M1 and M2). Both methods found average width to have increased, though this increase was smaller as measured by M1 than by M2, with statistically significant difference between values (p<0.05). M1 and M2 provided significantly different measurements for first and second interpremolar and first intermolar distances (p< 0.05). The values obtained using M1 (palatal landmarks for teeth) present less variation and provide information on the changes in the dental arches without adding tooth tipping. Considering M1, the greatest gain in transverse dimension occurred in first and second interpremolar distance, there was a slight increase in intercanine and first intermolar distance, and a decrease in second intermolar distance (AU)


Conocer la mejor manera de evaluar los cambios en el diámetro transversal de las arcadas producidos con el tratamiento ortodóncico permite estimar el espacio que puede generarse y optimizar la planificación del mismo. El objetivo del trabajo fue comparar las modificaciones del diámetro transversal utilizando reparos palatinos (M1) y reparos dentarios, (M2) luego de la alineación dentaria con sistema autoligable pasivo. Se evaluaron los maxilares superiores de 12 pacientes de ambos sexos (24 modelos pre y post tratamiento en total) atendidos con Sistema Damon en la Cátedra de Ortodoncia de la FOUBA, de entre 15 y 24 años de edad con apiñamiento moderado (discrepancia de Nance entre 4 y 6 mm). Se tomaron 5 medidas por arcada: distancia entre caninos, primeros y segundos premolares y primeros y segundos molares de los modelos pre y post alineación por tratamiento ortodóncico, utilizando dos métodos de evaluación (M1 y M2). La variación en el diámetro transversal promedio aumentó en ambos métodos y fue menor utilizando M1 que M2, con diferencia estadísticamente significativa entre los valores (p< 0.05). En el análisis de la distancia entre piezas dentarias homólogas, comparando M1 y M2, se encontró diferencia estadísticamente significativa entre primeros, segundos premolares y primeros molares (p< 0,05). Las arcadas presentaron incremento del diámetro transversal que fue evidenciado por ambos métodos de medida. Los valores tomando reparos en palatino de las piezas dentarias presentan menos variación e informan sobre las modificaciones de las arcadas dentarias sin agregar las inclinaciones de las piezas. Considerando M1 la mayor ganancia de diámetro transversal se produce en primeros y segundos premolares, con aumento leve en caninos y primeros molares y disminución entre segundos molares (AU)

OBJECTIVES: To determine the association between diabetes and the clinical features and treatment outcomes of TB in Kiribati. METHODS: We enrolled consecutive patients with TB who presented from August 2010 to February 2012 and compared clinical features and TB treatment outcomes for patients with and without diabetes, as measured by haemoglobin A1c assay. Poor outcome was defined as death, default or treatment failure, and good outcome as treatment success or cure. RESULTS: Two hundred and seventy-five eligible persons with TB disease were enrolled; 101 (37%) had diabetes. TB patients with diabetes were more likely to have acid-fast bacilli (AFB) seen on sputum smear microscopy (RR: 1.3; 95% CI: 1.03-1.62). The risk of poor outcome did not differ between patients with or without diabetes (RR: 1.1; 95% CI: 0.5-2.7). CONCLUSION: TB patients with diabetes are more likely than those without to have sputum with AFB on microscopy. This could increase transmission in the community. Early detection of TB by screening patients with diabetes, and the converse, could be important public health interventions where diabetes and TB are prevalent.
